Pleomorphic adenoma is the most common salivary gland tumour. The potential to undergo malignant transformation is rare but well described. The authors of this article present a case report of metastasis where the preceding submandibular gland pleomorphic adenoma preserves its benign phenotype. A brief literature search reveals that a review by Knight et al in 2016 found that only 81 cases have ever been reported where histologically, the pleomorphic adenoma retain their benign nature yet demonstrate malignant behaviour.
